RiseCare is a specialist community service from Western Medical Services, providing rapid, compassionate support to older adults following a fall or wellbeing concern.
Our clinicians don’t just help people back onto their feet. We assess, support and signpost, helping individuals remain safely at home wherever appropriate while providing reassurance to families and carers.

Rapid attendance following a non-injury fall to safely assist and assess the individual.
Clinical Assessment
Professional assessment following a fall or wellbeing concern to determine the most appropriate next steps.
